Summary:
The Nurse Educator provides education and evaluation to direct care staff in the performance of delegated nursing tasks. The Nurse Educator collaborates with the training department to facilitate medication certification training, and other health-related training courses, for employees. The Nurse Educator coordinates all hospital/rehab admissions/discharges and completes a post hospitalization review. The Nurse Educator serves as a leader and assists the nurse managers to ensure the safe, effective, and productive treatment of individuals at all times. This position could create performance-enhancing learning opportunities for staff and to ensure quality care for the individuals CDS Monarch supports.
Essential Job Functions:
+ Coordinate with the Training Department to provide or coordinate required education and training to staff including medication certification, new employee orientation, CPR/First Aid, and other specific health-related needs.
+ Assist with on-boarding and mentoring of new nursing staff.
+ Consult with Manager(s) of Nursing to provide additional training and hands-on skills practicums to nursing and direct care staff, including assisting with completion of medication practicums for staff who require new certs and annual certs.
+ Remain aware of changes and requirements for OPWDD and other regulatory bodies and assists in maintenance and revision of clinical policies and procedures as needed.
+ Assist in maintaining all required documents per Agency policy and procedures.
+ Complete targeted reviews and internal peer audits for the purpose of identifying and managing quality issues as assigned. Complete random validations
+ Coordinate all hospital/rehab admissions and discharges, including scheduling hospital discharge planning meetings.
+ Complete post hospitalization discharge checklist following all hospitalizations and shares finding with the assigned Nurse.
+ Assist with Nursing coverage of groups homes/day habs, as requested by leadership, due to planned or unplanned Nursing absences.
+ Serve on assigned committees (IRC, MRC, MERC, etc.).
+ Participate in the identification of complex medical cases to ensure coordination of quality care.
+ Adhere to all agency and program policies and procedures.
+ Perform all other necessary duties relevant to the position as requested by the supervisor.
Knowledge, Skills, and Abilities:
+ Ability to work independently and motivate others.
+ Ability to communicate effectively, both orally and in writing.
+ Proficient in time-management skills.
+ Proficient computer skills, including experience with Microsoft Office and EHR software.
Education and Experience:
+ Bachelor of Science in Nursing required
+ Must be licensed and currently registered as a Registered Nurse by the State of New York State Education Department
+ Specialized training or one year of professional experience in treating those with developmental disabilities preferred.
+ Prior training/teaching experience in related field preferred.
+ Must be at least 18 years of age.
+ Reliable transportation (NYS Drivers’ Licensed required).
All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Chief Executive Officer, in collaboration with Human Resources.
Physical Requirements/Working Conditions:
+ Sedentary working environment
+ Ability to reach above shoulder level.
+ Ability to turn/twist upper body.
+ Able to use hand repetitive action for fine manipulating, keyboarding and typing.

The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting.
