The Care Partner-LPN responsibilities include:

+ Works in collaboration with and under indirect coordination and oversight of the RN Care Partner Coordinators to assist with contacting patients and providers telephonically to improve the health, quality of health care services and experience of the populations served by the network.

+ Reviews daily Excel reports, reaching out to patients and providers and supporting the efforts of BayCare Physician Partners.

+ Performs other duties as assigned.

+ Potential travel within the BayCare service area using own transportation.

Position details:

+ **Location:** Hybrid

+ **Status:** Full time; 40 hours per week

+ **Schedule:** Monday - Friday 8:00 AM - 4:30 PM

+ **Weekend Requirement:** None

+ **On Call:** No

Certifications and Licensures:

+ Required LPN (Licensed Practical Nurse) License – State of Florida

+ Required Driver’s License – State of Florida

Education:

+ Required Technical Diploma – Licensed Practical Nurse

Experience:

+ Required 1 year as LPN

+ Required 3 years in healthcare

+ Preferred Experience in Inpatient or Outpatient Case Management
