UPMC Williamsport is seeking a Clinical Pharmacist to join our Emergency Department team. In this dynamic role, you will be responsible for managing daily pharmacy operations and providing direct clinical support within the Emergency Department. This is a full-time (40 hour work week) position, shift being 11AM - 10PM with every other weekend rotation and 2 holiday shifts per year.

Clinical Pharmacists ensure safe, effective, and efficient medication management for specialized patient populations. They provide direct patient-centered care, collaborate with interdisciplinary teams, and support both centralized and decentralized medication-use systems. Responsibilities include optimizing medication therapy, preventing adverse events, educating patients, and integrating clinical pharmacy services into the care process. Clinical Pharmacists also contribute to research, quality improvement, and education for pharmacy students and residents.

Key Responsibilities

Clinical Leadership & Program Management

* Lead assigned patient service lines, clinical areas, and therapeutic programs.

* Design and implement stewardship activities and restriction/surveillance programs.

* Track and evaluate pharmacy programs for operational, quality, and financial efficiency, benchmarking against best practices.

* Develop and oversee policies for drug purchasing, usage, distribution, and control.

Direct Patient Care

* Deliver patient-centered care in collaboration with multidisciplinary teams.

* Identify high-risk patients and implement interventions to improve safety and outcomes.

* Make evidence-based medication recommendations and manage medical emergencies.

* Provide discharge medication review, reconciliation, and counseling.

* Monitor patient profiles to prevent or resolve drug-related problems, including improper selection, dosing errors, adverse reactions, and treatment failures.

Operational & Pharmacy Services

* Participate in pharmacy operations, including medication dispensing and specialty medication procurement (e.g., chemotherapy, parenteral nutrition, controlled substances).

* Maintain proficiency in hospital systems and medication ordering platforms.

* Oversee technician activities and ensure compliance with operational standards.

Education & Practice Advancement

* Serve as a preceptor for pharmacy students and residents, developing training experiences and competencies.

* Facilitate experiential education and mentorship opportunities.

* Identify, design, and implement improvements in medication-use systems to enhance safety, optimize outcomes, and control costs.

Quality & Compliance

* Support and/or lead quality initiatives aimed at improving clinical pharmacy services.

* Monitor processes, analyze data, implement interventions, and evaluate effectiveness.

* B.S. degree in Pharmacy or Doctor of Pharmacy (PharmD) from an ACPE accredited College of Pharmacy (PharmD preferred)

* Current licensure as a Pharmacist in practicing state

* Completion of ASHP accredited PGY-1 Pharmacy Residency OR 3 years clinical pharmacy experience

* Knowledge and expertise in advanced pharmacy practice and service, comprehensive medication management, and evidence-based medicine in specialized area appropriate for clinical assignments.

* BPS Board Certification preferred.
