Job Duties Clinical

+ Respond to emergency situations and complete required documentation

+ Provide backup as Shift Leader and/or Rehab Tech duties as needed

+ Address patient issues ( Assessments, direct patient care, special tracking log)

+ Ensure communication of specific concerns regarding patient safety or health

+ Monitor and assist with dispensing of medications , as needed

+ Provide patient/care-giver education for medical conditions and medications

+ Communicate with On Call or/or physicians/office nurse , as needed

+ Model, teach and mentor the Rehabilitation Technicians on the Pate Core Values and Customer Service Quality Standards

+ Review and update as needed MAR's, PRN lists, and/or procedure records

+ Monitor patients vital signs, weights, hydration, output, food, intake, labs, blood sugar, responsiveness, skin integrity, etc, to ensure medical stability

Responsibilities

+ Supervise, educate, and develop Shift Leaders and Residential staff

+ Ensure timely and thorough documentation of care provided within the facility

+ Provide exceptional customer service, including patient/caregiver education and communication with patient care team

+ Ensure adequate supplies are available for all facility staff

+ Maintain a working knowledge of standards and regulations for THHSC, CARF, CRS, and CDC

+ Other duties as assigned by location

Qualifications

+ Supervisory experience preferred, but not required

+ Excellent verbal and written communication skills required

+ Must pass all pre-employment and employment verifications
