Hospice Medical Social Worker (LMSW or LLMSW/LiMSW)

Your Responsibilities:

+ Conducts initial and ongoing **hospice psychosocial assessments** for patients and families.

+ Collaborates with the **interdisciplinary hospice team** to develop, implement, and update the Plan of Care.

+ Provides skilled social work interventions in accordance with physician orders, professional standards, and hospice regulations.

+ Communicates changes in patient condition, safety concerns, and care needs to leadership and care team members.

+ Supports patients and families with counseling, education, and access to community and financial resources.

+ Maintains accurate, timely documentation and ensures compliance with **CMS, Medicare, and payer requirements** .

+ Implements strategies to promote patient safety and reduce avoidable hospitalizations or ER visits.

+ Upholds Elara Caring’s mission while maintaining strict **HIPAA privacy and confidentiality** standards.

What You’ll Bring

+ Master’s or Doctoral degree in Social Work from a **CSWE-accredited** program

+ **Current LMSW or LLMSW/LiMSW** , per state licensure requirements

+ Minimum of **1 year of social work experience in a healthcare setting** (hospice experience preferred)

+ Strong assessment, counseling, advocacy, and care coordination skills aligned with professional social work standards

+ Ability to travel locally (approximately **50% field-based travel** )

+ Valid driver’s license, reliable transportation, and current auto insurance
