    CHMC –Canton Health Center–Outpatient Clinic Care Coordinator– Full Time - Day Shift–Requisition 9135

     

    Pay Range: $17.45-$22.25

    Job Summary:

    The Outpatient Clinic Care Coordinator will be responsible for coordinating patient’s care with other providers, and communicating information as needed. They will cover for other clinical staff as needed. Assist with chart preparation for multiple clinics. They will also be responsible for preauthorization for services and testing, order tracking and scheduling. They will provide coverage for voicemail boxes and return patient messages. Arrange for and provide patient education. Will connect patients with community resources.

    Education:

    + High school diploma or equivalent

    Training and Experience:

    Required:

    + Possess strong critical thinking skills and clinical competency.

    + Ability to be self-motivated, independent, professional, creative, and dependable.

    + Ability to work with others as a team to ensure quality patient care and efficiency.

    + Familiarity with disease management, best practices, and current treatment regimens.

    + Basic computer knowledge.

    + Maintain confidentiality of all patient information.

    + Maintain quality assurance and compliance with all regulatory agencies.

    + Ability to implement and support procedures and programs pertaining to patient safety and security.

    + Exhibit proficient communication skills and ability to deal efficiently and effectively with a variety of personalities.

    + Ability to sustain attention to detail and accuracy as it pertains to the monitoring and documentation of information.

    Preferred:

    + Knowledge of disease management principles.

    + Motivational interviewing experience.
