Promotes wellness and disease prevention through education, special programs, and assessments

Responsibilities:

1. Works with patients to develop self-help plans and ensures that patients adhere to their established goals.

2. Coaches patients through individualized health plans by continuously encouraging healthy habits.

3. Reports on patient health status and progress. Develops and conducts wellness training courses.

4. Ensures that wellness programs meet customer needs and satisfaction.

5. Uses knowledge and skills to contribute to the development of functional objectives and principles to achieve goals in innovative ways.

6. Problems addressed are of diverse scope and complexity ranging from moderate to substantial.

Requirements:

+ Bachelors Degree or equivalent combination of education and related experience.

+ 5-7 years of relevant experience and progressive experience where team leadership was demonstrated, required.

The salary range for this position is $64350-$86060/year
