Schedule: Monday-Friday 9 am-5 pm
Job Summary : The Nurse Scheduling Coordinator is responsible for coordinating, scheduling, and confirming home care visits for Field Nurses providing skilled nursing services in patients’ homes. This role ensures appropriate nurse placement based on patient needs, physician orders, and nurse availability. The Coordinator serves as a central point of communication between patients, nurses, physicians, and internal staff to ensure continuity of care, compliance, and quality service delivery.
Responsibilities :
+ Coordinate and schedule Field Nurses based on patient care needs, active physician orders, and nurse availability
+ Maintain ongoing communication with nurses regarding assignments, visit confirmations, attendance, and schedule changes
+ Ensure all physician orders and care plans are active and accurately documented prior to scheduled visits
+ Manage nurse schedules using HHA eXchange, including monthly service calendars and real-time updates
+ Confirm completed visits and process time-sensitive field paperwork for weekly payroll submission
+ Monitor nurse attendance, missed visits, late visits, and call-outs; arrange coverage as needed
+ Serve as a liaison between patients, families, nurses, and physicians to support quality home care services
+ Answer incoming phone calls and communicate with patients, families, physicians, and staff in a professional and reassuring manner
+ Maintain accurate documentation while ensuring patient confidentiality and HIPAA compliance
+ Communicate staffing challenges, scheduling concerns, and difficult-to-staff cases with the Director and internal team
+ Participate in on-call rotation with other scheduling team members
Requirements:
+ High School Diploma or equivalent (required)
+ HHA eXchange experience (preferable)
+ Fluency in Spanish required
+ Prior experience in home care, healthcare scheduling, or care coordination preferred
+ Strong verbal and written communication skills
+ Excellent organizational skills with attention to detail
+ Ability to prioritize tasks and manage time effectively in a fast-paced environment
+ Professional, positive, and team-oriented attitude
+ Strong customer service skills with a patient-focused approach
