Position classification
Hybrid -- Job duties and expectations allow for onsite and remote work scheduled every week. Employees in this classification are regularly scheduled onsite one (1) to four (4) days per week based on the County needs and as determined by Department Director. Hybrid classified roles can be onsite more than the set minimum based on employee preference.

What Success Looks Like In This Job
This position is in Adams County Health Department (ACHD) Division of Strategic Health Initiatives (SHI). SHI prevents disease and advances fair health outcomes through community-based health promotion, policy and systems changes. Reporting to the Access to Care and Community Integrated Planning Manager, the Community Integrated Planning Senior Advisor plays a pivotal role in leading collaborative, data-drive, and community-centered assessment and planning efforts that inform ACHD’s strategic priorities. This position provides advanced public health planning and project management expertise to guide the development and implementation of the ACHD Community Health Assessment (CHA), Community Health Improvement Plan (CHIP), and Community Engagement (CE) Plan, ensuring shared leadership and share accountability across all divisions of ACHD as well as other partnering Adams County departments.
In partnership with ACHD’s Epidemiology and Data Science Division, and under the strategic direction and oversight of the Access to Care and Community Integrated Planning Manager, this position is responsible for coordinating county-wide and regional planning processes that strengthen partnerships, advance fair health outcomes, and authentically embed community voice into decision-making. The Community Integrated Planning Senior Advisor also serves as a liaison with internal and external partners, including community organizations, hospitals and health systems, and local and state agencies, to ensure alignment, integration, and shared leadership and accountability in community health planning and evaluation efforts.
Examples of Duties for Success
Leadership and Public Health Advanced Practice:
+ Serves as the project manager for the design and implementation of the department-wide CHA, CHIP, and CE Plan process, ensuring integration of evidence-based and community-informed methodologies as well as best practices for collective impact and change management
+ Serve as a subject matter expert and provide technical assistance across the department in community health assessments and planning, data analysis, evaluation and survey design, and population health improvement frameworks
+ Collaborate with the Performance Management and Quality Improvement Team to develop and manage performance measures and evaluation tools that monitor quality of and progress toward CHIP and CE Plan goals and inform department-wide progress
+ Assesses current state of various health and social determinants topics, translates public health data and community insights into actional recommendations for policy and systems change, and designs opportunities to collaborate within ACHD and align with community identified priorities through a collective impact approach
+ Supports alignment of ACHD CHA/CHIP and CE Plan with the ACHD Strategic Plan, priorities of collaborating Adams County departments and broader Adams County communities, and statewide public health improvement priorities
+ Provides capacity-building support and technical assistance to ACHD programs on community-integrated planning and evaluation practices
+ Effectively manages resource development and resource allocation, including grant writing and the management of budgets and contracts
Collaboration and Community Engagement:
+ Cultivates and sustains partnerships with community-based organizations, residents, local jurisdictions, health systems, and cross-sector partners (including other Adams County departments) with humility and in culturally responsive ways to ensure community-integrated health planning
+ Facilitate community engagement strategies that center community voice, lived experience and expertise, and accessible participation opportunities to ensure decision-making process and approach that fully involve impacted communities.
+ Uses public health and community engagement models to identify community health needs, priorities, assets and gaps.
+ Facilitates collaborative models for community health improvement using both collective impact and process improvement facilitation to leverage existing community efforts and strengths.
+ Represents ACHD on regional and state committees to coordinate community health assessments and identify shared priorities
Communication and Facilitation:
+ Designs and facilitates planning sessions, community forums, workgroups, and data collection opportunities such as focus groups that support and encourage authentic community engagement and consensus-building
+ Effectively translates and presents complex public health data, strategies, findings, and recommendations through meaningful narratives to various audiences, including community partners, ACHD leadership, County leadership, and other elected officials to support advocacy, transparency, and informed decision-making
+ Manages multiple projects at one time by using project scopes of work and tracking tools to maintain accountability. Maintains productivity standards and practices effective time management and prioritization of tasks.
+ Maintains active working knowledge of public health advanced practice, including the social determinants of health, community outreach and partnership development, population-based health promotion, disease prevention, coalition building, and collective impact.
+ Compiles and interprets data, makes recommendations on quality improvement; and assists with design and implementation of systems/programs.
+ Creates clear and accessible reports, presentations, dashboard, and other communication tools to share progress and outcomes with interest-holders.
+ Models effective communication that promotes trust, collaboration, and shared accountability among internal and external partners

Additional duties:
+ Performs other duties as assigned and required.
+ Responds, as required, to support public health emergencies, incidents, and events.
+ Participates in all emergency preparedness exercises and drills, as required.
+ Completes emergency preparedness and response trainings, as appropriate, as appropriate, within the health department.
+ Maintains sensitive & confidential client information according to the HIPAA confidentiality requirements.

Knowledge of:
+ Social determinants of health, their impact upon health, and strategies to address them within community settings.
+ Public health surveillance data and how to use them to plan and evaluate public health efforts.
+ Techniques and best practices related to community engagement, coalition building and developing community partnerships.
+ Community needs assessment, program development, and evaluation techniques.
+ Population health policy, prevention, and systems change strategies as well as community planning elements that protect public health and facilitate healthy behaviors and access to basic services
+ Microsoft operating system computer, Internet and spreadsheet functions and resources.
+ Colorado Public Health Assessment and Planning Systems (CHAPS)
+ Strategies and tools to effectively lead teams through complex project management, change management, and the application of complex policy and systems change frameworks to advance community-driven initiatives
More Qualifications for Success
Education & Experience:
+ A master’s degree in public or environmental health, public policy, sociology, planning, evaluation, or a related field AND three (3) years of related experience with progressively increasing responsibility,
OR
+ A bachelor’s degree in the same fields and five (5) years of related experience with progressively increasing responsibility.
Special Qualifications:
+ Must be proficient in use of online meeting platforms and Microsoft Office tools including Word, Excel, PowerPoint, and Outlook.
+ Reliable transportation is required to get to different community meeting locations as needed.
+ Bilingual preferred.
+ Must pass a criminal background check
Working Conditions and Physical Requirements
Work is primarily conducted indoors with occasional time spent in an outside environment. Incumbent will experience routine exposure to hazards of a typical office environment. Requires travel between Adams County offices and to locations in the surrounding community for investigations and meetings. May be required to work outside regular hours to attend meetings or events.
