Care Coordinator
Full Time | Monday - Friday | Temporary (Through March 2026)
Pay rate: $45/hour!
United Methodist Communities (UMC) in Cape May County, a 5-Star, full-service senior living community located in beautiful Ocean City, NJ, is now seeking a Care Coordinator.
The Care Coordinator is responsible for driving healthcare occupancy, managing post-acute discharge processes, and supporting continuity of care for residents transitioning into and out of the community. This role actively promotes referrals for long-term care and rehabilitation while building strong relationships with hospitals, providers, and community partners.
The Care Coordinator also provides post-discharge follow-up, including home visits and care coordination to ensure successful transitions and positive outcomes.
Care Coordinator Responsibilities:
+ Gather, evaluate, and review referral information to assess potential residents for admission
+ Make admission decisions and collaborate with nursing leadership as needed
+ Build and maintain a strong referral network, including onsite visits with partners
+ Facilitate the healthcare admission process, ensuring all required documentation is completed accurately and on time
+ Verify Medicare coverage and other insurance benefits
+ Participate in care conferences and interdisciplinary meetings as required
+ Conduct post-discharge home visits for sub-acute residents, including:
+ + Medication reconciliation
+ Psychosocial assessments
+ Follow-up support to ensure resident safety and well-being
+ Complete departmental reports accurately and within required timelines
Care Coordinator Requirements:
+ Bachelor's degree or higher from an accredited college or university in a health-related or behavioral science field
+ Minimum one (1) year of compensated or internship experience working with older adults or individuals with disabilities in a community or institutional setting
+ One (1) year of experience in case management or healthcare marketing
+ Knowledge of Microsoft Office
+ Familiarity with REPS (preferred)
+ Case Manager Certification
+ New Jersey Registered Nurse (RN) license
+ Licensed or Certified Social Worker
