    Description and Qualifications

     

    The Donor Family Aftercare Coordinator assists with meeting the needs of programs regarding post donor case bereavement support to donor families.

    ESSENTIAL JOB FUNCTIONS

    General Responsibilities

    + Becomes knowledgeable about organ, eye, and tissue donation and the transplantation field.

    + Provides bereavement and grief resources to families.

    + Facilitates correspondence between donor families and recipients.

    + Participates in the planning of donor family events and celebrations.

    + Helps with planning of special projects for donor families.

    + Collaborates closely with the Director, External Affairs and members of the Aftercare team.

    + Adheres to quality standards and legal compliance with regulatory and accreditation agencies (AOPO, OPTN, CMS, etc.).

     

    Bereavement Support

     

    + Delivers grief and bereavement support for LifeCenter Northwest donor families.

    + Assists with all post donor case family follow-up, including required mailings, phone calls, written correspondence, answering donor family questions, etc.

    + Ensures donor families have accurate, real-time access to information about bereavement support via our website, printed collateral, email, and over the phone.

    + Takes lead responsibility for facilitating donor family and transplant recipient communication in collaboration with transplant centers and other Organ Procurement Organizations, ensuring timely response to requests, and quality documentation.

    + Takes lead responsibility for organizing (including external communications) and facilitating monthly Correspondence Workshops offered to donor families and recipients served in LifeCenter’s donation service area.

    + Takes lead responsibility for requesting and managing recipient outcomes from transplant centers to facilitate sharing this information with donor families.

    + Provides support in coordinating the donor family quilt program and managing inventory.

     

    Takes part in virtual support groups and in-person supportive gatherings as is available.

     

    Communications and Events

     

    + Provides support to the Director, External Affairs and Donor Family Aftercare Events Specialist on the execution of donor family events.

    + Monitors the Donation Community Facebook group and supports website auditing.

     

    Documentation and Data Management

     

    + Ensures accurate and timely documentation of all family support interactions including, but not limited to, iTransplant.

    + Provides database management support to Aftercare programs.

     

    Supervision

     

    + This position has no supervisory responsibilities.

    Other Responsibilities

    + Participates in, and may lead, employee committees and events.

    + Complies with applicable laws, regulations, and LifeCenter policies and procedures.

    KNOWLEDGE, SKILLS, ABILITIES

    + Ability to learn and understand applicable local/state/federal regulations.

    + Crisis intervention skills and a demonstrated ability to discern between grief issues and other life issues that merit referral to other resources are necessary.

    + Multi-tasking, i.e., ability to function and complete work with competing priorities and expectations, good problem-solving skills.

    + Personal and interpersonal skills, i.e., clarity, diplomacy, consideration, sensitivity, confidentiality, and the ability to collaborate.

    + Excellent written and verbal skills, including the ability to prepare correspondence, manuals, and presentations with minimal supervision.

    + Ability to analyze and assess situations and create solutions.

    + Advanced knowledge of personal computers with at least intermediate and preferably advanced skills in Outlook, Word, Excel, MS Project, Power Point, and other comparable software programs.

    + Travel is essential to this position; consequently, the employee must be prepared to travel by automobile and airplane.

    + This position requires the ability to effectively operate phones, computers, and other office equipment.

    + The individual must have the ability to communicate clearly in English to accurately convey information.

    + Specific vision abilities required by this position include close vision, depth perception, and the ability to adjust focus.

    + Generally working conditions do not require exposure to hazardous materials nor extremes in health and safety hazards. Work is normally performed in an office or hospital setting, as well as other business environments.

    + Move equipment up to 25 pounds with or without assistance.

    ANTICIPATED TIME COMMITMENT

    The duties of this position will typically require 37 hours per week. Because of the nature of this position, the employee may need to work outside of regular business hours, including evenings and weekends, sometimes without advance notice.

    EDUCATION, EXPERIENCE & JOB REQUIREMENTS

    + + Bachelor’s degree from a four-year college or university is preferred.

    + Relevant work experience can be considered and substituted for academic requirements.

    + Documented courses/programs in grief and bereavement counseling preferred.

    + Minimum 1-2 years of experience in a related field such as counseling, chaplaincy, or social work preferred.

    + Experience in a health care environment or related field is preferred.

    + Requires appropriate documentation that allows the employee to travel by air (e.g. current real ID or passport).

    + Proof of COVID-19 vaccination or applicable medical or religious exemption will be required as a condition of employment upon offer.

LifeCenter encourages all employees to find balance and well-being and provides all employees access to an Employee Assistance Program (EAP) and provides a quarterly Wellness Reimbursement. Employees enjoy paid holidays throughout the calendar year and earn 119 hours of PTO annually (to start) as well as Health Time Off hours that is earned 1 hour per 30 worked, paid living donor leave, two weeks paid jury duty leave after 6 months of service, and three weeks of paid parental leave after one year of service is complete.     The pay range for this role is provided above. Where a candidate’s compensation falls within this range is based on several bona fide factors such as experience, tenure, and other specialized knowledge. LifeCenter Northwest is committed to providing its employees equitable and competitive compensation.

     

    This position is eligible for compensation in addition to the base pay in the form of overtime.
