Responsibilities:
To provide timely, comprehensive, and evidence-based nutritional care and education to patients with endocrine disorders and their families in the outpatient setting. Actively participate in the training of medical staff, dietetic interns, and peers. Ensure best practices by regularly reviewing current endocrine and nutrition literature and updating policies and practices as warranted. Deliver care in a manner that is highly responsive to patients’ needs and supports the highest quality of endocrine-focused nutritional management.
ESSENTIAL FUNCTIONS
Conducts comprehensive nutrition and diabetes self-management assessments for patients in an outpatient setting, including evaluation of medical history, lab data, medications, and lifestyle factors. Develops and implements individualized nutrition and diabetes care plans based on evidence-based guidelines, patient goals, and cultural preferences. Provides education and counseling on medical nutrition therapy, carbohydrate counting, insulin-to-carb ratios, food-drug interactions, and other aspects of diabetes management to patients, families, and caregivers. Adjusts plans as needed to support ongoing progress. Develops and updates patient education materials and resources for diabetes and nutrition management.
Effectively communicate nutrition plans and rationale to team members both verbally and in the medical record. Demonstrate the ability to utilize various HIPAA secure patient communication modalities such as “Zoom”, “My Chart”, etc.
Collaborates with interdisciplinary team members (physicians, nurses, pharmacists, behavioral health providers) to coordinate care and optimize patient outcomes. Provide lectures to residents, NP’s, and Fellows on diabetes and related endocrine conditions. Demonstrates advanced clinical skills and knowledge base and mentors Clinical Dietitians. Serves as clinical preceptor to dietetic interns and provides didactic training to nutrition students, peers (journal club) and other medical professionals (including medical students, surgical residents, fellows, attendings). Participates in orienting, training and in-servicing professional and other nutrition staff. Maintains expertise by pursuing pertinent continuing education opportunities.
Exercises independent judgment and displays advanced decision making. Evaluates literature, new products, and techniques; develops standards/protocols to ensure consistent, appropriate, evidence based therapies.
Participates in quality improvement initiatives and contributes to program development, evaluation, and reporting for outpatient diabetes education services. Represents the department at community nutrition functions. Attend regular department staff meetings as scheduled.
Research/Publishing/Speaking - Opportunities may arise and participation is encouraged, though not required.

MINIMUM EDUCATION & EXPERIENCE
+ Bachelor of Science degree in Nutrition, Foods and Nutrition Service or equivalent, completion of one-year dietetic internship experience or equivalent.
+ Master’s Degree in related field, preferred
+ Or equivalent combination of education and experience
LICENSES AND CERTIFICATIONS
+ Registered by the Commission on Dietetic Registration, the credentialing agency of The Academy of Nutrition and Dietetics Advanced certification, CNSC, CDCES, CSP or other specialist certification in specific area of practice required within 18 months of hire.
