Description
**PeaceHealth is seeking a Dir Palliative and Hospice Care for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $66.08 – $99.12.** The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
The Director is responsible for hospice (inpatient and community) and palliative care (inpatient, outpatient and community care locations) program development and optimization across PeaceHealth. In collaboration with the Home & Community Leadership Team (HCLT), Ambulatory and Acute leaders will develop long and short-term business and payer strategies, operational plans and supports implementation of key operational, clinical, and business strategies supporting the palliative and hospice services line.
Essential Functions
+ In partnership with HCLT, Ambulatory and Acute leaders, establishes monthly, quarterly, and annual program quality, operational and growth goals. Ensures alignment with PeaceHealth organization wide strategic goals.
+ Leads efforts to achieve and/or exceed determined goals for quality, operations, and program growth.
+ Ensures program compliance with all organizational, state, and federal laws, regulations standards and organizational policies and procedures. Ensures ongoing survey readiness.
+ Maintains partnerships with key stakeholders such as leadership, project sponsors, etc. at the system, network, and community level.
+ Establishes goals and metrics for hospice and palliative care across PeaceHealth, in partnership with key stakeholders and HCLT. Monitors goals and metrics and makes improvement recommendations to the communities.
+ Ensures the coordination and management of data. Tracks data, analyzes, trends outcomes and identifies opportunities for improvement. Ensures reporting on findings to key stakeholders.
+ Partners with learning and development to ensure that appropriate trainings are available to PeaceHealth caregivers.
+ Leads and participates in departmental meetings and is responsible for monthly reports of project progress as well as presenting outcome metrics. Participates in industry leadership collaborations and national associations.
+ Develops presentations and written communications; partnering with key stakeholders at all levels of the organization to champion the Home and Community strategic plan(s).
+ Performs other duties as assigned.
Qualifications
Education
+ Bachelor's Degree Required: and
+ Master's Degree Preferred: or a combination of education and professional experience that provides the caregiver with the requisite knowledge, skills and abilities to successfully perform the job
Experience
+ Minimum of 7 years Required: Experience in a healthcare setting; preferably community-based care, hospital, clinic, or health system related and
+ Minimum of 5 years Required: Related leadership experience, preferably in a healthcare setting; hospital, clinic, or health system related and
+ Required: Experience with managing complex programs and projects, specifically with a focus on customer service/service excellence/patient experience activities and demonstrated successful completion of quality improvement projects with desired outcomes.
Skills
+ Demonstrated teaching proficiency, including knowledge/application of Adult Learning principles. (Required)
+ Ability to deliver financial results for areas of accountability. (Required)
+ Proven skills in team facilitation, project implementation, effective data interpretation, customer relations. (Required)
+ Excellent verbal and written communication skills. (Required)
+ Ability to prioritize and manage multiple projects in a highly matrixed and disbursed organization. (Required)

+ Working knowledge of palliative care as well as general analysis tools and concepts (validity, reliability, etc.). (Required)

Working Conditions
Lifting
+ Consistently operates computer and other office equipment.
+ Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.
+ Sedentary work.
Environmental Conditions
+ Predominantly operates in an office environment.
+ Some time spent on site in medical/hospital setting.
Mental/Visual
+ Ability to communicate and exchange accurate information.
+ The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
