    Job Summary and Responsibilities

     

    This is a PRN position

     

    As a Nurse Practitioner with CommonSpirit Health at Home, you’ll provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patient’s continuing eligibility for palliative care.

    Responsibilities:

    + Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility.

    + Establishes a plan of care for continued needs based on the client assessment.

    + Reports changes in patient condition to appropriate personnel and the attending physician.

    + Provides patient care education, counseling, medication advice, instructions and follow-up to clients and other caregivers.

    + Participates in coordination of services with other disciplines and community resources as needed.

    + Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.

    + Assists with the development of educational programs in the palliative care program for diverse age groups.

    + All other duties as assigned.

    Job Requirements

    + Graduate of an accredited school of nursing.

    + Currently licensed to practice in the state serviced, with Masters in nursing.

    + At least 1 year of nursing experience

    + Must obtain and maintain current CPR

    + Strong critical thinking, clinical expertise, case management, and a passion for patient care.

    + Valid driver’s license and auto insurance coverage according to the CHI Health at Home policy.

    Pay Range

    $47.4107 - $70.52 /hour
