Job Details
HUB Support Associate:
Responsible for providing support to the - Care coordination and Denials Management Departments functions and staff, include the clerical support for Manager of Care coordination and Social Workers in facilitating patient's transitions to the next level of care,
Specific Responsibilities and Duties:
Care coordination Department:
+ Answers incoming telephone calls to the department. Takes a complete message and forwards call or message to appropriate staff.
+ Assists with concrete transition planning, including ordering supplies, transportation arrangements, DME, faxing/emailing related correspondence to facilities and agencies.
+ Assists with obtaining authorization for post-acute services when required.
+ Assembles and facilitates paperwork for QIO expedited appeals.
+ Maintains department files (paper and electronic).
+ Responsible for sending and receiving department faxes.
+ Distributes census, reports required for Care coordination staff.
+ Orders supplies / equipment, oversees office needs and performs various duties as required.
+ Performs duties as required to support the daily functions of the Care coordination Department.
+ Manage the Hub Support team’s productivity and performance.
+ Train new team members and provide necessary coaching if needed.
+ Escalate insurance authorization delays to payer leadership and or hospital leadership

Skills
+ Proficient in the use of a personal computer - including standard office software Word , Excel ; experience with basic file and directory management required; experience with spreadsheet and database software and / or mainframe systems preferred.
+ Proficient in the use of office equipment such as faxes, copiers , scanners.
+ Demonstrated ability to work with minimal supervision once instructed and proficient in required activities.
+ Ability to effectivel y communicate in a verbal and written manner; demonstrate ability to
be organized , neat and detail oriented.
+ Demonstrates a courteous and professional demeanor , team spirit and the ability to work with office staff and all hospital personnel in a collaborative manner.
Knowledge
+ Knowledge of medical terminology preferred.
Position Requirements and Qualifications
+ High School degree required. Two years college experience or associate’s degree preferred.
+ Hospital or medical office experience preferred
