    The primary function of this role is to support quality patient care at Apollo Home Healthcare in partnership with the Care Team and Director of Patient Care Services. You will leverage your experience in home health nursing to support field clinicians and direct home health services, ensuring that patients receive care in accordance with the agency's policies, procedures, and best practices. By joining our team, you will work alongside compassionate and dedicated healthcare professionals who are committed to delivering the highest level of care to our members.

    You will be involved in the following areas:

    + Handling incoming calls from patients and staff regarding clinical questions. If additional information is needed, you will escalate the call to the Director of Nursing and ensure a prompt response.

    + Collaborating with physicians, other healthcare providers, and community agencies to ensure coordinated care.

    + Analyzing clinical data and identifying opportunities for improvement.

    + Promoting a positive and supportive work environment for clinical staff

    + Supporting and educating new and existing staff to ensure quality patient care.

    + Providing oversight and care coordination for designated caseload of patients.

    + After hours/on call responsibilities to support the team

    + Reviewing and signing Start of Care Orders, recertification orders, resumption of care orders, discharge orders and external orders.

    + If local and office-based: Providing patient visits on an as-needed basis to support continuity of care and meet fluctuating caseload requirements.

    + If local and office-based: Shadowing and training clinicians and completing skills checklists in the field

    + Other duties as assigned by supervisor

    Requirements

    + Active California RN BSN License

    + A minimum of 1-2 years of experience with Home Health

    + Current CPR Certification

    + Proficiency in wound care, wound vac, IV, blood draw, foley/suprapubic catheters, ostomy care (colostomy, nephrostomy), feeding tubes

    + We are open to candidates who would be in-office, hybrid, or fully-remote for this role.

    Preferred Qualifications:

    + Knowledge of intake, scheduling, authorizations, OASIS and orders.

    + killed in Kinnser or an equivalent Home Health EMR

    Salary

     

    The annual salary range for this role is $90,000-$110,000. Actual compensation will be informed by candidate’s location as well as confirmed job-related skills and experience.
