Case Manager
As a Case Manager in our Community Support Program, you work directly with adults living with mental illness to help them build meaningful, self-directed lives in the community. Through supportive counseling, service coordination, and advocacy, you help individuals access resources, strengthen daily living skills, and navigate complex systems of care. Working closely with internal teams, psychiatrists, housing partners, and community providers, this role is central to promoting recovery, stability, and long-term wellness. Case Managers typically support a caseload of approximately 20–30 individuals, depending on level of need.
Job Duties (Summary)
+ Providing support services to clients such as outreach visits, supportive counseling, support learning independent living skills, and transportation
+ Helping clients to remain responsible for their own lives
+ Will work closely with housing coordinator and other internal and external partners
+ Provide treatment plans and updates
+ Advocate for clients when they are not capable of doing this for themselves
+ Attend required meetings and trainings, including individual and/or group supervision and other agency committee work when indicated
+ Providing your supervisor and other team members with the information necessary for them to ensure the efficient delivery of services
+ Completing all required paperwork on time
+ Fulfilling other duties appropriate to your position as assigned
Qualifications
Education
+ Bachelor’s degree in social work or a related mental health field with a minimum of one year of relevant experience required.
+ Master’s degree in social work or a related mental health field preferred.
+ Supervision toward mental health or social work licensure is provided.
Knowledge and Competencies
+ Experience providing services to individuals living with mental illness in community-based settings.
+ Strong interpersonal and communication skills, with the ability to build therapeutic relationships.
+ Ability to coordinate services across multiple systems and providers.
+ Knowledge of recovery-oriented, person-centered, and strengths-based practices.
+ Strong organizational skills and ability to manage a diverse caseload.
+ Effective advocacy skills and ability to support individuals in navigating complex systems.
+ Ability to maintain professional boundaries, confidentiality, and ethical standards.
Work Environment
This position is primarily community-based and includes time spent in offices, client homes, and community settings. Case Managers travel locally to meet with individuals, attend appointments, and collaborate with partners. The role involves regular computer use for documentation and communication, as well as participation in meetings and supervision. Flexibility and independence are essential, as schedules are shaped by client needs and service demands.
Compensation and benefits:
This Full-time, exempt position starts at $24.87 and includes comprehensive benefits including paid time off, employer-sponsored health, dental, life, and disability insurances; a 403(b) retirement plan with generous employer match; and educational benefits such as tuition reimbursement and student loan repayment.
