Remote Registered Nurse (RN) Home Health Case Manager

Are you a highly organized and experienced Registered Nurse (RN) with a knack for detail and a passion for patient care? Acute Home Healthcare is looking for a dedicated Case Manager to join our dynamic home health team. In this pivotal role, you will be the administrative and clinical anchor for our patient care, ensuring that every case is managed with precision and efficiency.

About the Role

This is not a traditional field nursing position; instead, you will be the central hub of communication and documentation for our home health cases. Your primary responsibilities will include, but are not limited to:

+ Reviewing RN's OASIS Assessments to ensure completion and accuracy.

+ Recording case information and completing all necessary forms with accuracy.

+ Assisting with case management, including drafting service plans, reviewing case progress, and determining case closure.

+ Facilitating multiple care aspects, including case coordination, information sharing, and discharge planning.

+ Ensuring requests for visits are filled out and sent to appropriate staff.

+ Following up on patient cases and overseeing all patients to ensure care is initiated in an efficient and timely manner.

+ Reviewing clinical records to ensure services have been provided in accordance with the patient's plan of care (POC).

+ Ensuring physician orders are signed.

+ Communicating with the PCP and/or insurance company regarding changes in condition, the need for new/modified orders, etc.

+ Meeting with direct care staff and administrative staff to communicate any follow-ups or concerns regarding patients.

+ Holding and documenting case conferences with other members of the healthcare team.

+ Assisting with verifying notes for billing and verifying appropriate documentation resides in the patient's clinical record.

+ Developing effective working relationships with the clinical team throughout the lifespan of the case management process.

+ Ensuring all deadlines are met and adhering to professional standards as outlined by protocols, rules, and regulations.
