Responsibilities
It is the purpose of the Recreational Aide to assist the Activities Program Director in providing social, recreational and other purposeful activities to each resident in the Nursing Care Residence in order to stimulate the resident's mental, physical and sensory capabilities to their fullest extent. The incumbent enables the residents to foster a sense of usefulness, self-respect and dignity in maintaining their quality of life. The incumbent serves as a resource person for information regarding the resident and to encourage and motivate their participation in these programs to their fullest capacity. The incumbent performs a variety of hands on activities including, crafts, cooking, recreational and social activities with the use of community resources in large and small group programs and one on one, in and outside the Facility. Adheres to Facility Standards of Conduct and practices the Core Values of Service of the Poor, Reverence, Integrity, Wisdom, Creativity and Dedication. Represents OLP accurately and honestly. Acts with integrity and in the best interest of OLP. Identifies instances of non-compliance and takes appropriate action, including reporting violations of the CRP. Adheres to Facility policy or regulations. Business relationships are conducted in the best interest of OLP, free of gifts, favors or other improper inducements.
Position Accountabilities and Performance Criteria
The following are essential job accountabilities:
1) Transport residents to and from the activity rooms. Lead daily activities as directed. (20%).
+ Application of Knowledge Performance Criteria - Complies with Universal Precautions appropriate to their position while performing all job duties in order to minimize exposure. Assures that established safety regulations are followed at all times. Utilizes proper lifting and body mechanics when performing all duties to prevent personal injury. Demonstrates competency to transport residents outside the Facility.
+ Record Keeping Performance Criteria - Documentation of daily attendance at all activity programs.
+ Resident Relations Performance Criteria - Demonstrates the core value of creativity by providing a variety of hands on activities including crafts, bingo, cooking, exercise, recreational and social activities with the use of community resources in large and small group problems.
2) Monitor any changes in a resident's condition and reports changes to the appropriate staff immediately. (20%).
+ Application of Knowledge Performance Criteria - HIPAA Competency- Protects resident's privacy by insuring any items with resident data on them are not left unattended. Maintain confidentiality of all resident information ensuring their rights are protected. Identifies emergency situations and directs immediate care.
+ Patient Relations Performance Criteria - Communicates pertinent information regarding resident's needs to appropriate nursing home staff.
+ Record Keeping Performance Criteria - Reports accurate results to achieve the best possible resident care. Prepares accurate and timely documentation during all shifts of duty as required by guideline protocols.
+ Resident Relations Performance Criteria - Recognizes changes in resident's condition and asks appropriate questions to clarify the situation. Reports changes to appropriate personnel.
3) Assist in the planning and passenger list for regular outings.(15%).
+ Application of Knowledge Performance Criteria - Competency: Knowledge of the proper methods of constructing correspondence, memos and reports, including correct grammatical usage. Maintains a positive attitude in work place; works well with and assists others as needed to complete the job.
+ Planning/Timeliness Performance Criteria - Participates in offering suggestions for outtrips for the residents to keep them active in the community.
+ Record Keeping Performance Criteria - Organizes work to complete assignments within required timeframe.
4) Present information in one on one and small group situations to customers, clients and other employees of the organization.(16%).
+ Application of Knowledge Performance Criteria - Demonstrates attitude of cooperation and professionalism when working with all staff members, serving as a resource to co-workers and flexibility in working hours and days scheduled in order to meet the needs of the Facility. Effectively identifies predictable problems and adjusts available resources to facilitate positive learning. Strives to incorporate the philosophy and mission of the Facility. Maintains confidentiality of all resident, Facility and/or physician related information. Maintains resident confidentiality and does not discuss residents in public access areas, such as cafeteria, hallways, elevators, etc.
+ Communications Performance Criteria - Ability to effectively present information to others. Must establish and main good personal and public relations. Works as a team player for the benefit of the facility, department, co-workers and residents.
5) Perform one on one room visits for psychosocial and sensory stimulation to the room bound residents. (10%).
+ Communications Performance Criteria - Communicates technical information in a manner appropriate for the intended audience.
+ Management Performance Criteria - Effectively identifies problems, researches all available options and makes appropriate decisions/recommendations.
+ Planning/Timeliness Performance Criteria - Organizes work to complete job tasks in a timely manner. Responds promptly to change in workload, adjusting assignments to achieve optimal productivity and efficiency.
6) Drive the facility van or bus to take residents on outtrips to keep involved in the community as assigned by the Director. (6%).
+ Application of Knowledge Performance Criteria - Complies with departmental dress code; wears Facility ID badge while on duty; reports to work well groomed and in compliance with infection control standards for person hygiene. Wears appropriate attire including street clothes which may be worn when assigned to an out trip excluding the following: Jogging pants, sweat suits or jeans. Competency: Attends yearly driving skills inservices as needed or requested by the facility and/or Activity Director. Watches a driving skill video and takes a written driving test regarding the video on a yearly basis. Demonstrates safe driving skills. Demonstrates and is observed for good driving technique by the facility maintenance department and/or Director. Demonstrates proper wheelchair lift safely. Insures that the properpaperwork such as DNR, closet care plans, etc. are obtained for every outtrip.
+ Communications Performance Criteria - Reports any problems/concerns regarding outtrips, repairs ormechanical issues regarding the van or bus.
+ Planning/Timeliness Performance Criteria - Has and maintains a clean NYS drivers license (without blemish) with good driving and vision skills.
+ Must produce DMV Driving Abstract yearly, or sooner if requested and turns into the Director, a copy of which will be referred to Human Resources.
+ Record Keeping Performance Criteria - Signs an outtrip attendance record listing residents who attended the outing. Signs off on who was the driver/watcher for each out trip. Maintains and turns in receipts and individual resident envelopes at the end of every outtrip.
+ Resident Relations Performance Criteria - Maintains a positive attitude and remains a good marketing tool for the facility while on outtrips.
7) Attend all inservices and meetings as required.(4%).
+ Application of Knowledge Performance Criteria - Competency: Demonstrates appropriate phone skills and use while on outtrips. Competency: Participates in drills and demonstrates knowledge of Plan for Department. Insures no more than one file drawer in a cabinet is opened at any one time. Insures file drawers are closed when not actively being used. Projects poise, authority, and a positive image. Wears appropriate clothing and accessories, giving proper attention to personal hygiene. Wears Facility ID Badge while on duty. Infection Control Competency: Demonstrates knowledge of and adheres to all infection control procedures including proper hand washing techniques, contact with blood spills and universal precautions. Attends all mandatory inservices. Attends department meetings as required and scheduled.
The following duties may be reassigned:
8) Provide assistance in the planning of communication including announcements, bulletin boards, invitations and the monthly activity calendar and distribution. (3%).
+ Application of Knowledge Performance Criteria - Reports to work on time and ready to work at the start of the shift; provides proper notification when late or absent. Demonstrates flexibility to work varied hours including evenings, holidays and weekends. Demonstrates attitude of cooperation and professionalism when working in department. Completes job duties within required timeframe, according to established schedules or workflow requirements and assists co-workers to do the same.
+ Collaboration Performance Criteria - Maintains community involvement for residents.
+ Planning/Timeliness Performance Criteria - Responds promptly to inquires and requests, notifies department staff to matters requiring their attention.
9) Assist in planning, developing, implementing and evaluating the activity program.(2%).
+ Application of Knowledge Performance Criteria - Maintains orderliness and cleanliness of work area. Returns equipment and tools to proper location after use, clean and in good condition.
+ Management Performance Criteria - Recommends to the Director the equipment and supply needs of the department.
+ Planning/Timeliness Performance Criteria - Obtains information concerning each resident's background for use in initial assessments and review assessments, including correct grammatical usage.
+ Record Keeping Performance Criteria - Follows the resident care plans for pursuit of activities. Maintain a daily record for each resident's attendance in the program.
+ Resident Relations Performance Criteria - Promotes socialization, recreation and enjoyment with each resident's physical, mental, cultural and ethnic needs.
10) Assist with resident specific functions as needed.(2%).
+ Application of Knowledge Performance Criteria - Utilizes proper lifting and body mechanics to prevent personal injury. Demonstrates/cooperates by assisting with resident feeding following successful completion of feeding assistant training program.
+ Collaboration Performance Criteria - Assists in the seasonal/holiday decorating of the units. Assists in the set-up and breakdown of the activities programs.
11) Assist with shopping, errands for activity supplies and residents' personal needs.(2%).
+ Application of Knowledge Performance Criteria - Demonstrates ability to count money accurately.
+ Collaboration Performance Criteria - Coordinates activities with other departments as necessary.
+ Planning/Timeliness Performance Criteria - Demonstrates appropriate paging and phone skills.
12) Perform other related duties as assigned or requested.
Requirements
Licensure / Certification / Registration:
+ Current valid, clean NYS Driver's License (without blemish) with good driving and vision skills required
+ Preferred Registration/Certification: NYS Nurse Aide Certification or COTA preferred
Education:
+ High School or equivalent. Employees in job classification prior to 1/22/2010 are grandfathered.
Work Experience:
+ Minimum Experience: On the job training.
+ Preferred Experience: One (1) year experience in a clinical health care field, recreation or acceptable position as determined by management.
Additional Preferences
Part Time 15 hours weekly
+ Current valid, clean NYS Driver's License (without blemish) with good driving and vision skills required
+ Preferred Registration/Certification: NYS Nurse Aide Certification or COTA preferred
