Job Overview

The **Field-Based Case Manager** is responsible for coordinating and managing care for elderly members and individuals with disabilities who are approved for in-home or nursing facility services. This clinical role uses a collaborative, member-centric approach to assess needs, develop care plans, coordinate services, and advocate for optimal, cost-effective outcomes. The position is primarily field-based with significant travel to members’ homes and remote work in between visits.

Key Job Duties

+ Conduct comprehensive, in-person and telephonic assessments using clinical tools, data review, and member interviews

+ Evaluate member eligibility, benefit plans, and available internal/external programs and services

+ Develop, implement, and monitor individualized case management plans

+ Apply and interpret clinical criteria, guidelines, policies, procedures, and regulatory standards

+ Coordinate care across providers, community resources, and support services

+ Advocate for members to ensure appropriate utilization of benefits and services

+ Utilize case management and quality management processes in compliance with regulatory and accreditation requirements

+ Maintain accurate documentation in multiple systems and proprietary platforms

+ Collaborate virtually with interdisciplinary teams while working independently in the field

Clinical Role Requirement

+ **Required Licensure (Illinois – unrestricted & active):** LCSW/LCPC/RN

Experience Required

+ **3–5 years** of clinical experience preferred

+ **2–3 years** of care management, discharge planning, home health, or care coordination experience preferred

+ Experience working with elderly populations and individuals with disabilities strongly preferred
