    Care Manager

    Job Description

    We are seeking a dedicated and compassionate Care Manager to join our team. This role involves managing a caseload of program members, collaborating with an RN Care Manager, and providing comprehensive assessments and care plans. The position is part of a mission-driven organization focused on helping elderly individuals and adults with disabilities live independently.

    Responsibilities

    + Manage an assigned caseload of program members in collaboration with an RN Care Manager.

    + Complete comprehensive member assessments, member-centered care plans, and functional screens.

    + Provide ongoing care coordination and quality assurance monitoring of services.

    + Maintain required documentation in adherence to established standards.

    + Provide direct social work case management services.

    + Facilitate team management and maintain documentation standards.

    + Regular local travel within Racine county, requiring use of a personal vehicle, valid driver's license, and proof of insurance coverage.

     

    Skills & Qualifications

     

    + 1 to 2 years case management experience with the elderly and/or developmentally or physically disabled population preferred.

    + Bachelor’s Degree in a Human Services or closely related area required.

    + A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.

    + Experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity preferred.

    + Master’s degree in Social Work preferred

    + Current Wisconsin Social Work licensure preferred

    Work Environment

    The position is based in Milwaukee with telework currently being the norm. Work hours are Monday to Friday, 8:00 AM to 4:00 PM, including travel within Racine county. A valid driver’s license is required.

    This is a Contract to Hire position based out of Racine, WI.

    Pay and Benefits

    The pay range for this position is $23.00 - $35.00/hr.
