Your Impact in This Role

+ Conduct in-home evaluations for speech, language, cognition, voice, and swallowing

+ Develop and implement individualized treatment plans per physician orders

+ Educate patients and caregivers to support progress between visits

+ Collaborate with nurses, therapists, and physicians to ensure coordinated care

+ Complete timely, accurate documentation

+ Promote patient safety and help reduce avoidable hospitalizations

What You’ll Bring

+ Master’s degree in Speech-Language Pathology from an accredited program

+ Active state SLP license (or Educational Limited License with supervision)

+ Prior SLP experience preferred; home health or hospice experience a plus

+ Strong time management and independent decision-making skills

+ Compassionate, patient-centered approach

+ Reliable transportation and valid driver’s license
