Speech-Language Pathologist (SLP) – Home Health

Full-Time | 4-Day Work Week

Territory: Bay City • Midland • Saginaw

What You’ll Be Doing

+ Perform in-home evaluations of speech, language, cognition, voice, and swallowing

+ Develop and carry out individualized treatment plans aligned with physician orders

+ Educate patients and caregivers to support progress between visits

+ Collaborate with nurses, therapists, and physicians to ensure continuity of care

+ Maintain accurate, timely documentation

+ Promote patient safety and help reduce avoidable hospitalizations

What You’ll Bring

+ Master’s degree in Speech-Language Pathology from an accredited program

+ Active state SLP license (or Educational Limited License with supervision)

+ Prior experience preferred; home health or hospice experience is a plus

+ Strong organizational skills and ability to work independently

+ Compassionate, patient-first mindset

+ Reliable transportation and valid driver’s license
