    Position Status:Full Time

     

    FLSA Status: Non-Exempt

     

    Pay Rate: The pay range for this role is generally$52,000-$57,000annually, commensurate with relevant experience and qualifications and in alignment with internal equity.     Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

    POSITION SUMMARY:

    The primary role of the Harm Reduction Coordinator is to facilitate the daily processes of Community Health Action’s Harm Reduction programming on our Mobile Health Unit serving multiple communities throughout Staten Island. Core to this program’s service menu is street-level outreach and engagement of individuals managing an opioid use disorder. The Coordinator will work in a fast-paced environment in collaboration with other Harm Reduction managers and direct service staff implementing harm reduction services targeting active substance users and their significant others. The mobile harm reduction work includes, but is not limited to, the following services: community canvassing to conduct outreach and engagement, sterile syringe exchange, harm reduction education and counseling, and opioid overdose prevention services.

    DUTIES & RESPONSIBILITIES:

    + Provide direction and oversight of the day-to-day operations of services offered through the mobile health unit’s harm reduction programming.

    + Work to increase the range and utilization of harm reduction services. Ensure that persons engaged with Senior Peer Mentor, Volunteer Peers, and other harm reduction staff have access to a full range of harm reduction services.

    + Ensure program performance meets requirements as defined in contract documents, programmatic workflows, and CHASI policies and procedures.

    + Provide direct supervision to Senior Peer Mentors. Collaborate with the Harm Reduction Program Director and Human Resources on recruitment, orientation, training, and evaluation.

    + Recruit Peers to engage community members in harm reduction objectives. Provide ongoing training and development, and support to peers in fulfillment of role.

    + Monitor Peer client level interactions (timeliness, frequency, and appropriateness), and chart documentation to support meaningful engagement.

    + Ensure accurate and timely documentation of client engagement in agency’s electronic health record and funder databases. Maintain program data in secure storage and ensure staff adherence to applicable confidentiality regulations and standards.

    + Develop, manage, and monitor systems to support the timely dispatch of Sr. Peer Mentors and Volunteer Peers to designated program locations/events.

    + Develop Peer work schedules and ensure leave or vacancy coverage plans in collaboration with harm reduction team; provide on-call supervisory coverage on a rotating basis and provides consultation to staff and Peers to address emergent or urgent issues they encounter in the community.

    + Conduct Quality Assurance activities and develop Quality Improvement projects in compliance with program standards.

    + Function as the “Site Administrator” for the Mobile Health Unit and field-based locations, working closely with the Harm reduction Director and Operations Department to address any facility needs; assist with site-based programming as needed.

    + Prepare timely reports for internal and external stakeholders, including the funder.

    + Participate in supervision, Harm Reduction team mtgs, CHASI’s Management Team meetings, workgroups, and committees.

    + Represent CHASI in conferences, taskforces, and coalitions. Participate in conferences to foster communication between Peer Mentors and community partners.

    + Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.

    + Other duties as assigned.

    Requirements

    QUALIFICATIONS

    + At least 3 years of experience working with individuals with substance use disorder, preferably in a harm reduction focused program.

    + High School diploma or equivalency required; Associate degree may supplement for 1 year of experience; Bachelor’s degree may supplement for 2 years of experience

    + Certified Peer Recovery Advocate (CRPA) CRPA required. This can also be obtained within the first 90 days of employment.

    + At least 1 year of experience supervising staff and/or management experience.

    + Oriented to the scope of practice of peers.

    + Commitment to multiple pathways to recovery, including medication assisted treatment and harm reduction.

    + Commitment to client-centered service delivery.
