Occupational Therapist (PRN)

What You’ll Do

+ Evaluate patients to determine occupational therapy needs and develop individualized plans of care

+ Complete initial evaluations and ongoing reassessments in compliance with OASIS and PPS guidelines

+ Collaborate with physicians, caregivers, and interdisciplinary team members to ensure continuity of care

+ Implement patient-centered interventions focused on activities of daily living, adaptive equipment, sensory integration, and fine motor coordination

+ Educate patients and caregivers on home programs and therapy techniques

+ Assist with securing necessary equipment, including fabrication of custom splints when needed

+ Provide supervision and clinical direction to COTAs, HHAs, and other clinical staff as applicable

+ Maintain accurate, timely documentation in accordance with agency and regulatory standards

Physical & Work Requirements

+ Physically active, home-based work environment

+ Full range of motion, including lifting, transferring, and assisting patients

+ Ability to lift 50–100 pounds

+ Ability to travel within the assigned coverage area (up to 50%)

+ Potential for occasional irregular or on-call hours

What We’re Looking For

+ Graduate of an accredited Occupational Therapy program

+ Current, unrestricted Occupational Therapist license in the state of practice

+ Minimum of one (1) year of clinical experience preferred

+ Reliable transportation, valid driver’s license, and current auto insurance

+ Ability to sit, stand, bend, lift, and move intermittently throughout the day

You will report to the **Branch Director, Clinical Manager, Clinical Supervisor, or designee** and be supported by a leadership team committed to your success.
