Community Work Services (CWS), is seeking a motivated and mission-driven professional to join our Case Manager team. This role is a critical part in supporting individuals with disabilities and other barriers to employment by delivering person-centered case management, employer engagement, and community-based workforce services. The ideal candidate thrives at the intersection of human services and workforce development, excels at building relationships with employers and community partners, and is passionate about helping participants secure and retain meaningful, competitive employment.
Responsibilities:
+ Maintain manual and computerized participant records, complete required documentation and reporting including CWS database (FedcapCares) input with timeliness and accuracy.
+ Plan and coordinate off-site recruitment opportunities.
+ Network with other providers to generate community resources for participants.
+ Effectively partner with internal and external operations and business partners to meet the needs of employers and job-seeking participants.
+ Establish/maintain relationships with targeted community groups and/or individuals to maximize interest in CWS training programs.
+ Assist participants with researching and accessing needed services including additional job training, educational supports, counseling, housing, medical services, and other support services as needed.
+ Coordinate service provision with referring agencies including conducting case conferences at set intervals.
+ Determine individual's need for support funds/ gift certificates; help them investigate and exhaust other options before requesting funds.
+ Generate productive partnerships with businesses to allow for ongoing job /internship placement of job-seeking participants.
+ Monitor and follow-up with participants to enable them to find and retain employment.
+ Provide/assist with coordination of job coaching and follow-up retention services for participants working competitively.
+ Conduct tours of CWS for interested potential participants as well as prospective employer partners.
+ Act as the point person between different stakeholders to ensure satisfaction with services provided at all levels.
+ Perform follow-up calls and visits to employers to ensure a high level of employer satisfaction.
+ Provide case management, community-based activities, and individualized employment services for individuals referred through the Department of Developmental Services.
+ Attend individual service plan meetings and develop person-centered ISPs based on participant input, skills, interests and needs
+ Oversee implementation of ISP goals and document progress towards goals
+ Serve as liaison between Community Work Services and the Department of Developmental Services, families, residential staff, and guardians.
Education and Experience:
+ Bachelor's Level in Vocational Rehabilitation, Counseling, Social Work or other relevant area with at least 2 years' case management experience and at least 1 year working in a human service/community-based setting
+ Outstanding verbal and written communication skills
+ Experience working in and/or ability to demonstrate commitment to nonprofit work targeting economically disadvantaged populations; homeless, elderly, ex-offenders, low wage earners, individuals with disabilities, working families, un/underemployed and hard to place individuals, highly preferred.
+ High attention to detail, exceptional time management skills and strong organization skills
+ Ability to analyze situations, anticipate and/or detect problems and provide solutions
+ Aptitude to manage time and tasks efficiently with little supervision
+ Ability to think creatively and connect meeting business needs and social service goals
+ Ability to work in a fast-paced environment
+ Competitive, results driven personality with the ability to work well with others in a team
+ Proficiency in Excel, PowerPoint and other Microsoft Office tools
+ Bilingual (English-Spanish) a plus
+ Familiarity with Boston area businesses, providers, and resources required
