    Facility Hours

    _Shifts are scheduled within the following hours based on operational needs:_

    + **Monday:** 10:00 AM – 7:30 PM

    + **Tuesday–Thursday:** 6:00 AM – 7:00 PM

    + **Friday:** 6:00 AM – 7:00 PM

    + **Saturday:** 7:00 AM – 5:00 PM

    + **Sunday:** 8:00 AM – 3:00 PM

    Key Responsibilities

    + Welcome and engage patients to create a positive, supportive care experience

    + Accurately collect vitals and patient intake information

    + Provide patient education and reinforce after-care instructions

    + Complete documentation and required forms thoroughly and on time

    + Collaborate with providers and clinical staff to deliver high-quality, coordinated care

    Qualifications

    + Active Wisconsin LPN license

    + Strong communication and interpersonal skills

    **Pay Details:** $24.00 to $33.00 per hour
