Position Description
The **Director of Case Management** is responsible for the overall management and coordination of case management department activities within an acute care hospital setting. This leader ensures compliance with organizational policies and regulatory requirements while coordinating patient care and discharge planning processes across hospital departments and external agencies to promote continuity of care and optimal resource utilization.
This role provides oversight of all daily case management operations and is accountable for achieving established clinical, operational, and quality outcomes through collaboration with interdisciplinary teams and external stakeholders. The Director maintains executive-level presence and demonstrates strong communication, leadership, and performance management skills.
Job Duties
+ Direct and evaluate departmental operations, including case management models, staffing plans (skill mix and FTEs), onboarding, competencies, and use of information systems.
+ Allocate resources to ensure appropriate staffing levels and achievement of productivity and quality targets.
+ Oversee core case management functions and professional practice standards.
+ Ensure timely, high-quality care coordination and discharge planning with accurate documentation.
+ Collaborate with finance and senior leadership to meet established goals and operational expectations.
+ Monitor departmental performance through data analysis and implement sustainable performance improvement initiatives.
+ Ensure compliance with organizational policies, procedures, and regulatory standards.
+ Establish and maintain collaborative relationships with key stakeholders, including medical staff leadership, nursing leadership, ancillary services, ethics and compliance teams, and legal partners.
Qualifications
+ Bachelor’s degree required
+ Master’s degree in Nursing, Health Administration, or Business Administration preferred
+ Active licensure as one of the following:
+ Registered Nurse (RN)
+ Licensed Clinical Social Worker (LCSW)
+ Licensed Master Social Worker (LMSW)
+ Minimum **3 years of acute care hospital case management experience** , required
+ Minimum **2 years of case management leadership experience in an acute care setting** , required

**Pay Details:** $102,107.00 to $140,000.00 per year
