Essential Duties and Responsibilities:
1. Receives and reviews 24-hour report at the beginning of the shift.
2. Administers and is accountable for controlled drugs per state regulations and Momentum policies and procedures. Reports any discrepancies or errors to Registered Nurse immediately. Additionally, informs Home Administrator immediately AFTER notifying Registered Nurse (RN).
3. Conducts walking rounds to determine persons supported status every two (2) hours and as needed, includes beginning and end of shift.
4. Interacts with Direct Support Professionals (DSP), informing them of persons supported particular medical needs, treatments, precautions to be taken and direction and assistance as needed to provide optimum health care.
5. Schedules and coordinates appointments as needed, per location, for persons supported to maintain required medical care on an on-going basis.
6. Serves as a member of the Interdisciplinary Team at the Individual Service Conferences to assist in the development of the Individual Service Plan (ISP).
7. Prepares medications for one scheduled administration time. Passes medication according to Momentum policy and procedures which allows one (1) hour prior to and one (1) hour after scheduled medication.
8. Maintains medication and treatment supplies as needed, including faxing new treatment and re-orders to pharmacy. Receives items and checks in and distributes on a timely basis according to Regulations.
9. Communicates persons supported change of condition directly to physician when Registered Nurse is unavailable. Carries out phone orders, makes appropriate appointments, and informs Home Administrator, parents, guardians, Sr. Director of Nursing, and Regional Center when appropriate.
10. In the event of a serious incident, such as need to go to the hospital or Emergency Room, LVN must immediately notify parents, guardians, Home Administrator, Sr. Director of Programs, and Sr. Director of Nursing.
11. Ensures the medication cart is locked when not attended at all times. Only when the LVN/Med-tech/RN is preparing medication, stocking the cabinet, etc. is the medication cabinet open and LVN/Med-Tech/RN must remain with the medications until the cabinet is locked. Maintains cleanliness and organization of medication chart throughout shift.
12. Administers and records medication in a timely manner in Medication Administration Record (MAR) and progress notes as necessary.
13. Documents all treatments or nursing interventions entering into persons supported chart and/or Sandata/Carasolva during their shift.

14. Assists in initial evaluation of persons supported upon admission.
15. Informs Registered Nurse (RN)/ Sr. Director of Nursing, and Home Administrator of all changes in persons supported condition which may require further nursing/medical assessment and charts same in persons supported chart and/or Sandata/Carasolva.
16. Assists in dining rooms during meals, as needed. Monitors special diets, feeding, assisting/training staff in proper feeding techniques.
17. Completes incident reports when asked and gives to Home Administrator and Registered Nurse (RN) for final approval.
18. Assists the clinical portion of ANT classes as needed. Will provide constructive feedback to Administrator and Regional or Senior Director of Program Services.
19. Provides persons supported status updates in order for Registered Nurse (RN) to determine Day Program attendance.
20. Escorts persons supported to dental and medical appointments as need arises.
21. Assists doctors on monthly rounds to ensure safety and rights of persons supported as needed.
22. Weighs all persons supported and documents appropriately as ordered.
23. Communicates with other departments to ensure quality care.
24. Completes all nursing documentation on a timely basis and by the end of shift.
25. Understands and follows the facility fire/disaster procedures and participates in drills. Understands role to follow in case of any emergency.
26. Uses proper body mechanics and observes all safety precautions to minimize injuries to self and persons supported.
27. Practices, at all times, Momentums no lift policy and procedures.
28. Follows Health Care Plan as written by Registered Nurse.
29. Follows assigned Registered Nurse (RN) directions to ensure continuum of care is followed throughout their shift. Will report to Registered Nurse (RN) if directions were not followed and why.
30. Understands and follows the Universal Precautions and Infection Control procedures.
31. Understands and follows all Momentums procedures and policies.
32. Performs various other functions as assigned or as requested by Registered Nurse (RN).
33. Performs scheduled assignments involving Adult Day Program (ADP) as part of nursing responsibilities.
34. Informs Registered Nurse (RN) immediately of any significant occurrences and/or change in persons supported status.
35. Sandata/Carasolva to be accessed only during work hours, no exceptions.
36. Attends mandatory monthly staff meetings.
37. Performs other duties as assigned to meet department and/or business needs
