*Position Details:* Hartford Hospital is seeking a Physician Assistant or Nurse Practitioner to join their Department of Medicine team! We are looking for a non-clinical APP to conduct outreach post discharge calls tocomplex patients, ensuring discharge instructions are adhered to. Go over medication reconciliation lists and confirm prescriptions are filled and picked up.
