    Description:

    Summary

    The transitions of care (TOC) pharmacist is responsible to ensure that each patient served has a safe and optimized medication management plan at the time of discharge. To effectively optimize the medication management plan for each patient, the TOC pharmacist is expected to integrate themselves onto various care teams. At discharge, the TOC pharmacist will provide each patient with a standardized bundle of pharmacy services to reduce hospital readmission rates and improve patient outcomes. The TOC pharmacist must maintain accurate and up to date outcome data.

    Essential Functions:

    + Implement pharmacy bundle at discharge for at least 8-10 patients daily

    + Bundle components:

    + Detailed review of discharge medications to ensure no errors are present

    + If errors or optimizations are identified, intervene with discharging provider

    + Provide medication discharge counseling to patient and/or caregiver

    + Provide a follow-up phone call 24-72 hours after discharge to ensure no unforeseen barriers to medication management occurred

    + When necessary, assist with medication cost concerns through prior authorization, formulary adherence and/or copay assistance

    + Document completion of all bundle components in EPIC

    + Complete monthly metrics/reports to track readmission rates for patients who received pharmacist TOC services at discharge and those who did not

    + Collaborate with multidisciplinary teams to optimize workflow and patient outcomes

    + Example: Population health care coordinators, case management

    + Participate in the transitions of care pharmacy council and multidisciplinary readmission reduction committees where appropriate

    + Provide formal and informal education to pharmacists and providers on the value of transitions of care services

    + Identify and advocate for service line growth through bright ideas, completion of targeted continuing education, pharmacy advocacy and escalation of near misses

    + Adhere to all hospital and department policies

    + Precept pharmacy residents and pharmacy students

    Schedule:

    + Monday – Friday, 0830-1700

    Qualifications:

    + Bachelor’s degree or PharmD from an ACPE accredited pharmacy school

    + Completion of a PGY-1 pharmacy residency OR 3 years of work experience

    + Strong clinical skills to ensure implementation of optimized medication management plans

    + Strong communication skills with the ability to articulate information in a clear and concise manner, employ probing questions, and use of the teach back method

    + Demonstration of our Core Values

    + Self-motivated and able to work independently with minimal supervision

     

    Pay Range based on Experience and location $56.00-$81.60
