    Responsibilities:

    As part of an interdisciplinary and interagency, service delivery grant, the social worker collaborates to build community capacity, remove barriers to care and provide direct social work services to children ages 0-21 with diagnosed or suspected intellectual or developmental disabilities (IDD) identified through the work of the project. The social worker is an active member of the project team working to understand current systems of care and how they hinder access , and develop improved care models. Ongoing social work services provided to children and families affected by IDD who face substantial barriers to care will inform the work and contribute to improve outcomes for children.

    ESSENTIAL FUNCTIONS

    Collaborates with URMC clinicians and staff of various regional health care teams (primary care, specialty centers, mental health providers, care coordination systems, etc.) to optimize screening and identification of children with IDD, facilitate appropriate and timely diagnostic evaluation, reduce barriers to care, and ensure continuity and coordination of care between clinical settings.

     

    + Maintain ongoing relationships with key clinicians and staff in various health settings to facilitate appropriate transitions of care

    + Provide training to healthcare providers within URMC and across NYS to increase clinician skills in developmental screening and care of children with IDD

     

    Collaborates with regional systems of care with whom individuals with IDD interact (school districts and other educational systems, mental health, state and local disability services, county health and social services, etc.) to develop and institute systems of care that support individuals with IDD throughout their life.

     

    + Develops and maintains relationships with key community stakeholders in order to facilitate effective collaboration.

    + Provides training to individuals and groups across NYS to improve care of children with IDD

     

    Participates as a key member of the DBP intake team; consulting to members of the DBP/URMC team, consulting with health and educational providers, and conducting intake interviews/psychosocial assessments, in order to determine appropriate sources of care, and facilitate care in DBP or in other community-based settings.

     

    + Documents SWK activity in patient’s medical record

    + Communicates effectively with other members of the healthcare team to facilitate needed care

    + Schedules appointments in eRecord as needed

    + Facilitates collection of information needed for evaluation and addressing barriers to care associated with the intake process through work with community partners and child caregiver.

     

    Provides social work assessment/evaluation and risk screening for DBP and potential DBP patients/families identified by clinicians.

     

    + Completes psychosocial assessment of all patients identified at risk with the emphasis on caregiver adjustment to parenting a child with significant development issues, transitions in care, and connection with educational and community supports.

    + Documents SWK activity in patients’ medical record according to SWK division policy

    + Provides SWK services, referral and advocacy to patients/families. Serves as a consultant to patients and families regarding community resources, financial assistance and special programs (Medicaid waivers, Health Home, OPWDD)

     

    Identifies risk management issues and reports them to DBP leadership and SWK administration

     

    Identifies and manages cases of suspected child abuse and neglect in the DBP Center

     

    Provides back up coverage for another social worker as needed

     

    May provide field work instructions and supervision to SWK students

     

    Other duties as assigned

     

    Participates in activities of the SWK Division including; staff meetings, professional development, supervisory meetings, absence approval, time reporting and other mandatory trainings.

     

    Participates in administrative activities in DBP, may include staff meetings and program development

    MINIMUM EDUCATION & EXPERIENCE

    + Must hold a Master of Social Work degree from an accredited institution.

    + Experience working with an IDD population and or children with other medical complexities, Preferred

    + or equivalent combination of education and experience, Preferred

    KNOWLEDGE, SKILLS & ABILITIES (Preferred)

    + Demonstrated ability to work within an interdisciplinary/ interagency team.

    LICENSES AND CERTIFICATIONS

    + NYS licensure required within 18 months of employment
