What You'll Do
Essential functions include:
+ Flexibility in working in acute care capacities with adults and pediatrics ( central distribution, IV room, med/surg, and critical care)
+ Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards
+ Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems,
+ Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care
+ Makes appropriate evidence-based, patient- centered medication recommendations
+ Participates in the management of medical emergencies for adult and pediatric patients
+ Provides discharge medication review, reconciliation, and counseling as appropriate
+ Maintains proficiency in and actively engages in central dispensing functions and operational programs
+ Oversees pharmacy operations and technician practice/activities
+ Maintains proficiency in preceptor roles and actively emulates education/ mentorship skills
Preferred Qualifications
-Acute care clinical pharmacy practice experience with adult and pediatrics
-High sensitivity to pharmacy support and teamwork
-Flexibility with work schedule and hours
What We're Looking For
+ Education:
+ 4 Year / Bachelors Degree, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification
+ PharmD, preferred.
+ Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, preferred.
+ Licensure/Certification:
+ Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
+ Board of Pharmacy Specialties Certification, required.
+ Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
