    Job Summary and Qualifications

     

    Responsible for developing and coordinating initiatives directed at improving and assuring the safe use of medications within the organization. This includes developing and maintaining productive working relationships and providing support to other disciplines on which safe medication use is dependent, standardizing medication management practices, ensuring regulatory compliance, implementing national quality agenda related to medication management, medication safety programs, and staff development/education.

    Medication Use Safety

    + Develops a strategic plan to improve the safety of the medication-use system.

    + Assesses the medication management system for risk points and identifies strategies to improve safety.

    + Conducts at least one FMEA annually.

    + Coordinates the reporting and analysis of adverse drug reactions.

    + Coordinates the reporting and analysis of medication errors.

    + Develops policy and procedures that promote safe medication use practices.

    + Participates in the development, implementation, and maintenance of medication-use safety initiatives.

    + Effectively uses the appropriate analytic tools and process improvement techniques to assess and improve medication-use processes.

    + Coordinates with organizational leaders to assure that medication-use processes are conducted in a safe physical environment.

    + Coordinates the development and implementation of strategies to eliminate or minimize medication errors.

    + Assesses and reports medication use safety improvement activities to leadership and appropriate committees.

    + Ensures appropriate and safe use of medication-related automation including smart pumps, order entry systems, and automated dispensing machines.

    + Assesses published literature to identify best practices and/or guidelines relevant to medication-use safety and opportunities for reducing medication errors.

    Performance Improvement

    + Chairs Drug Safety Team and submits report of medication safety activities to the Pharmacy and Therapeutics Committee

    + Chairs Medication Error Team and submits report of medication safety activities to the Drug Safety Team, Patient Safety Team, and Pharmacy and Therapeutics Committee

    + Maintains a performance improvement program that improves medication safety throughout the organization

    Regulatory Compliance

    + Complies with federal, state, and local regulations and accreditation standards relating to medication- use safety.

    + Assesses and reports compliance with Medication Management accreditation standards.

    + Develops policy and procedures to meet regulatory compliance standards.

    + Reviews controlled substance waste and return records and reconciles discrepancies.

    + Assesses and documents compliance with corporate controlled substance monitoring requirements.

    + Monitors for controlled substance theft or diversion, assists with investigations and reporting requirements.

    Education and Training

    + Manages pharmacy staff development, competency and training in collaboration with the Pharmacy Clinical Coordinator and Pharmacy Supervisors.

    + Provides medication-use safety education to pharmacy staff, healthcare providers and other appropriate staff.

    + Provides orientation and training to new staff members.

    + Provides regulatory or accreditation compliance education to healthcare providers and other appropriate staff.

    + Provides education and training to pharmacy students.

    Additional Duties

    + Performs pharmacist duties when needed.

    + Demonstrates the ability to create, initiate, and integrate new concepts and practices to improve patient care and/or employee satisfaction.

    + Develops creative solutions that address departmental or organizational needs.

    + Behaves appropriately and works cooperatively with others to achieve departmental goals.

    + Responds to conflict discretely and appropriately.

    + Accept and supports change and new ideas.

    + Consistently puts the team's success ahead of personal recognition.

    + Performs work within specified time frames.

    + Performs work completely with attention to detail.

    + Attends staff meetings and participates in other programs, committees, meetings, and functions required by the hospital or pharmacy.

    + Assumes authority, accountability and responsibility for the formal and informal direction of others.

    + Performs other related functions as assigned.

    What qualifications you will need:

    + Doctor of Pharmacy or Master’s degree required. Pharmacy residency training or BCPS certification preferred

    + Registered pharmacist licensed in the State of Florida required

    + 3 years or equivalent health-system experience in a patient care related field required

    + Pharmacy management or supervisory experience preferred

    + Previous patient safety work or pharmaceutical redesign work preferred

    + Experience in project management and patient safety initiatives preferred

    + Presentation, research and publishing experience preferred

     

    HCA Florida Osceola Hospital is a 404-bed tertiary care hospital. We are accredited by the Joint Commission and are a Level II Trauma Center. We are a teaching hospital in collaboration with UCF College of Medicine. Our hospital is conveniently and centrally located in the Heart of Kissimmee.
