As the 1115 Enhanced Services Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:
+ Conduct screenings with Medicaid eligible individuals for services under the 1115 Waiver using the approved ACH screening. Ensure all demographic information is accurately collected and member consent is properly documented.
+ Deliver basic navigation services to eligible individuals, including eligibility assessments, resource referrals, and documentation of all services provided in the Unite Us platform.
+ Ensure timely and accurate data entry of all services, interactions, case notes, and outcomes in the Unite Us platform in accordance with 1115 Waiver requirements
+ Represent Catholic Charities at community events, partner meetings, and outreach initiatives to raise awareness about 1115 Waiver services.
+ Participate in all required trainings associated with the 1115 Waiver, including but not limited to: Unite Us platform, trauma‐informed care, culturally and linguistically appropriate services, HIPAA, and compliance.
1115 Enhanced Services Case Manager Minimum Salary: $22.00 per hour
1115 Enhanced Services Case Manager Maximum Salary:$22.00 per hour
1115 Enhanced Services Case Manager Schedule: 35 hours per week, Monday - Friday 8:30 AM - 4 PM
Qualifications:
+ Associate's Degree in Human Services or related field
+ 1-3 years experience with diverse population
