    Job Description:

    Shift: 11:00am - 7:30pm

    Schedule: Part-time (20 - 24 hours per week / flexible days)

    + Discusses facility core measures during handoff communication.

    + Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation.

    + Collaborates with interdisciplinary team members to solve admission and discharge problems.

    + Collects comprehensive patient medical history.

    + Establishes and maintains positive relationships with patients and their families.

    QUALIFICATION REQUIREMENTS:

    Associate's of Nursing (Required), Bachelor's of NursingBasic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Registered Nurse (RN) - EV Accredited Issuing Body

    Pay Range:

    $35.07 - $65.24
