    Overview

    Shift: Monday - Friday | 8:00 AM - 5:00 PM

    * This is a hybrid position. Will be expected to go into the Urbandale/West Des Moines office 2-3 days/week.

     

    Oversees the organization, accuracy, and integrity of the integrated health record system, ensuring all patient information is complete, confidential, and compliant with regulatory standards. Partners closely with clinical teams, administrative departments, and compliance to identify, communicate, and resolve documentation corrections across multiple information management systems. Ensures timely updates, document attachments, and follow-through to support patient safety, continuity of care, and effective care coordination.

    Responsibilities

    • Responsible for a wide variety of records processing functions such as phone coverage, processing record release requests, quick disclosure requests, sending records to storage or destruction of records after attached electronically and QA’d, retrieving records, performing QA checks to make sure records are complete, and sending incomplete documents to the clinical manager for completion.

    • Monitors and responds timely to all communication sent to Chart Correction team within the email box, and/or in-basket.

    • Responsible for locating and filing/scanning all medical records for patient visits according to office policies and procedures in a timely manner.

    • Corrects and resolves incorrect documentation issues within clinical systems such as multiple patients' information in one health record.

    • Acts as a liaison between the system Release of Information team and UnityPoint at Home for compliant record release

    Qualifications

    Education:

    + High School or Vocational School graduate.

    + All staff are expected to participate in appropriate continuing education as may be requested and/or required by their immediate manager. In addition, staff are expected to accept personal responsibility for other educational activiites to enahnce job-related skills and abilities. All staff must attend mandatory educational programs.

    Licensure/Certifications:

    + Valid driver's license when driving any vehicle for work-related reasons.

    Other:

    + Use of usal + customary equipment used to perform essential functions of the position.
