What You’ll Do

+ Deliver high-quality, in-home physical therapy services aligned with Elara Caring’s mission and standards

+ Evaluate patient functional needs, establish measurable goals, and adjust care plans to achieve optimal outcomes

+ Design and implement individualized plans of care with patient and caregiver involvement

+ Collaborate with interdisciplinary team members to support patient progress

+ Provide direction to support staff to help patients meet functional goals

+ Maintain accurate, timely documentation including evaluations, treatment plans, and progress notes

+ Serve as a clinical resource and advocate for physical therapy within the community

What You’ll Need to Be Successful

+ Graduate of an **APTA-approved Physical Therapy program**

+ Current, unrestricted **Physical Therapist license** in the state of practice

+ Minimum **one (1) year of experience** in a healthcare setting

+ Ability and willingness to travel within the assigned coverage area (up to 50%)

+ Reliable transportation, valid driver’s license, and current auto insurance

+ Ability to sit, stand, bend, lift, and move intermittently; must be able to lift **50–100 lbs**
