Job Description:

**Schedule:** Full-time, 36 hours per week

**Shift:** Mid-shift, 11:00 am - 11:00 pm (with weekend rotation)

Primary Job Responsibilities:

+ Conduct comprehensive assessments of patients' physical, psychosocial, environmental, and developmental health status.

+ Administer prescribed medications and treatments safely, monitor patient responses, and document all related activities accurately.

+ Document patient assessments, care plans, interventions, and outcomes in medical records to ensure continuity of care.

+ Educate patients and their families about health conditions, treatments, medications, and preventive care.

+ Collaborate with healthcare providers, specialists, and support staff to ensure cohesive and comprehensive care delivery.
