    Job Description:

    + Provides skilled nursing care to hospice patients in private homes, skilled nursing facilities, assisted living facilities, and inpatient units.

    + Assesses patient conditions and symptoms to determine the need for crisis care.

    + Administers medication, takes vitals, feeds, bathes, changes clothes and bedding, dresses wounds, and provides other palliative care as indicated in the care plan.

    + Offers support and education to the patient's family and caregivers.

    + Communicates with supervisory RN regarding symptom management, changes in patient condition, safety issues, and other patient care concerns.

    QUALIFICATION REQUIREMENTS:

    Technical/Vocational School (Required)Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Certified Hospice and Palliative Nurse (CHPN) - EV Accredited Issuing Body, Driver's License (DL) - EV Accredited Issuing Body, Licensed Practical Nurse (LPN) - EV Accredited Issuing Body

    Pay Range:
