    Job Description:

    Ensures clear and effective communication among healthcare team members.

     

    Provides timely updates to patients and families about their care and discharge plans.

     

    Coordinates with various departments to facilitate smooth transitions and avoid delays.

     

    Develops and implements efficient discharge planning protocols.

     

    Assesses patients' readiness for discharge and ensures all necessary paperwork and instructions are completed.

    QUALIFICATION REQUIREMENTS:

    Bachelor's of Nursing (Required), Master's of NursingBasic Life Support - CPR Cert (BLS) - Accredited Issuing Body, Certified Nurse Manager and Leader (CNML) - EV Accredited Issuing Body, Registered Nurse (RN) - EV Accredited Issuing Body

    Pay Range:

    $72,879.32 - $135,547.09
