    Job Description:

    + Conduct comprehensive evaluations of patients' physical, psychological, and social health to identify their needs.

    + Develop individualized, evidence-based care plans with specific, measurable goals tailored to patient needs.

    + Implement care plans by administering treatments, medications, and therapies as outlined. * Continuously monitor and assess patient progress towards achieving expected outcomes, document outcomes, and adjust care plans as necessary. Collaborate with other healthcare professionals to ensure a coordinated and comprehensive approach to patient care.

    + Act as a liaison to post-hospital care providers and community health resources to ensure continuity of care.

    + Oversee patient transitions through the healthcare system, ensuring smooth handoffs and transfer of accountability upon discharge.
