Job Description:

+ $3,000 Relocation available for eligible candidates (external only, 12-month contract required, must be relocation greater than 50 miles for the purpose of employment at CFD facility

+ Location: 600 Courtland Suite 110, Maitland, FL 32804

+ Travel up to 75%

+ Ensures timely communication with patients, their families, and healthcare providers through professional collaboration.

+ Collaborates with the Community Care team to evaluate patients' medical and psychosocial conditions and assess their needs.

Pay Range:
