Job Description:

Shift: M-F FT 8-5pm

+ Assesses patient’s physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records, and clinical data.

+ Evaluates patient’s response and reports to responsible provider to ensure optimal patient outcome.

+ Contributes to the development and implementation of patient care plans in collaboration with the interdisciplinary team.

+ Demonstrates competency in therapeutic and diagnostic interventions, including vital signs, minor treatments/procedures, prevention services, pre-visit planning, post-visit follow-up, and medication refills per protocol.

+ Promotes, advocates, and collaborates to protect the health, safety, and rights of each patient.
