    Job Description:

    Develops plans of care for psychosocial needs of patients. Assists patients and families with financial needs and resources. Provides counseling to patients and their families. Understands and addresses end-of-life issues. Works independently while maintaining case load expectations. Collaborates with interdisciplinary teams to implement and evaluate care plans. Conducts initial psychosocial evaluations and ongoing support. Refers patients and families to community resources as needed. Assists with Medicaid applications and placement in nursing homes or assisted living facilities. Performs other duties as assigned. Engages in community education and outreach activities. Supports patients and families in achieving desired outcomes through effective social work interventions.

    QUALIFICATION REQUIREMENTS:

    Bachelor's (Required), Master's (Required)Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Clinical Social Worker License (LCSW) - EV Accredited Issuing Body, Driver's License (DL) - EV Accredited Issuing Body

    Pay Range:

    $23.71 - $44.09
