    Job Description:

    + Consults with physicians, patients, hospital staff, family members, or significant others to resolve barriers to treatment progress and ensure continuity of care.

    + Maintains records of behavioral health interventions, assessment, diagnostic impression, and treatment for accurate statistics.

    + Makes timely referrals to Federal, state, county, and community agencies.

    + Develops relationships with agencies to ensure appropriate service delivery to patients.

    + Assesses patients for behavioral health needs and provides referrals as needed. Other duties as assigned.

    QUALIFICATION REQUIREMENTS:

    Master's (Required)Clinical Social Worker License (LCSW) - EV Accredited Issuing Body, Licensed Marriage and Family Therapist (LMFT) - EV Accredited Issuing Body, Licensed Professional Counselor (LPC) - EV Accredited Issuing Body

    Pay Range:

    $61,560.00 - $114,504.00
