    Job Description:

    + Reviews reports on Current Core Measure Hospitalizations and Re-admissions daily.

    + Coordinates the Readmission Prevention Committee Meetings, identifies recent readmission cases, reviews for improvement opportunities, creates and sends out meeting agenda, and completes meeting minutes.

    + Participates in in-patient visits and post-discharge phone calls, utilizing data reports to identify patients with Core Measure diagnoses.

    + Visits patients in the hospital, provides education on disease process and medications, and collaborates with Care Management to address discharge needs.

    + Collaborates with Respiratory Therapy and Cardiopulmonary Rehab Teams when appropriate.

    Associate's of Nursing (Required), Bachelor's of NursingRegistered Nurse (RN) - EV Accredited Issuing Body

    Pay Range:

    $68,132.50 - $119,520.35
