    Job Description:

    + Conduct comprehensive patient assessments to determine physical, psychological, and social health needs.

    + Develop individualized, evidence-based care plans with specific, measurable goals tailored to patient needs.

    + Implement care plans by providing direct patient care, including administering treatments and medications.

    + Monitor and evaluate patient progress, document outcomes, and adjust care plans as necessary to achieve desired health outcomes.

    + Educate patients and families about self-care techniques, disease management, and the use of medical equipment.
