    What We’re Looking For:

    The Forms Specialist is responsible for accurate, timely and compliant completion of health-related forms based strictly on the patient’s medical record. This role ensures all information transcribed _(_ entered, abstracted) is consistent with documented clinical facts, prepares and releases appropriate supplemental medical documentation, and adheres to HIPAA and organizational privacy standards at all times. This position requires exceptional attention to detail, the ability to efficiently navigate Electronic Medical Records (EMR), and a strong understanding of privacy, authorization and medical documentation workflows.

    What You Will Do:

    + Complete STD, FMLA, LTD, and other health-related forms accurately and consistently with the medical record, without interpretation beyond documented provider notes.

    + Extract and synthesize relevant clinical information (e.g., diagnoses, functional limitations, treatment dates, restrictions) from the medical record to support form responses.

    + Identify discrepancies or missing documentation and escalate appropriately for provider clarification when needed.

    + Navigate EMRs and related clinical systems to locate relevant notes, test results, and supporting documentation.

    + Prepare and compile supplemental medical documentation (e.g., visit notes, diagnostic reports) that supports completed health forms.

    + Ensure documentation released aligns precisely with what is authorized and requested.

    + Review and validate patient authorizations prior to releasing any protected health information (PHI).

    + Release medical records and supporting documentation in strict compliance with HIPAA, state, and organizational privacy requirements.

    + Apply minimum necessary standards when disclosing information.

    + Adhere to established policies, procedures, and regulatory requirements related to medical documentation and privacy.

    + Perform quality checks to ensure accuracy, completeness, and compliance prior to submission or release.

    What You Need to Succeed:

    + High school diploma or equivalent required; associate’s degree or higher in health information management, healthcare administration, or a related field preferred.

    + 2+ years of experience in healthcare documentation, medical records, disability forms processing, or a related role.

    + Demonstrated experience navigating electronic medical records (EMRs).

    + Strong understanding of HIPAA and patient privacy principles.

    + Exceptional attention to detail and accuracy.

    + Strong organizational skills and ability to manage multiple requests with competing deadlines.

    What Helps You Stand Out:

    + Experience completing STD, FMLA, and/or LTD forms.

    + Background in Health Information Management (HIM), Release of Information (ROI), or medical records.

    + Familiarity with payer, employer, and third-party disability documentation requirements.

    + Knowledge of state and federal leave and disability frameworks.

    Physical Requirements:

    + Remote work environment.

    + Prolonged periods of computer and EMR system use.

    + Handling of sensitive and confidential information.

     

    Pay ranges for this job title may differ based on location, responsibilities, skills, experience, and other requirements of the role.

    The estimated base pay range per hour for this role is:

    $16—$19 USD

    To ensure the safety of patients and staff, many of our clients require post-offer health screenings and proof and/or completion of various vaccinations such as the flu shot, Tdap, COVID-19, etc. Any requests to be exempted from these requirements will be reviewed by Datavant Human Resources and determined on a case-by-case basis.
