    SUMMARY

    As an Occupational Therapist, your strong attention to detail and excellent evaluation skills are crucial to successfully restoring or improving each patient’s optimal level of function, independence, and quality of life.

     

    STATUS: Per Diem

     

    DEPARTMENT: Occupational Therapy

     

    SCHEDULE: Day Shift 8-hour shifts; flexible start & end times

     

    Position Summary:Assesses and restores patients' function, alleviates pain, and prevents disabilities by designing, planning, and implementing medically prescribed occupational therapy interventions. Provide superior customer service by modeling the Brand Promise and Core Values.

    Key Responsibilities:

    Meets the patient's goals and needs and provides quality, evidence-based care by assessing, developing, and implementing an individualized treatment plan. Helps patient accomplish treatment plan and accept therapeutic devices and recommendations by instructing, encouraging, and assisting patients in performing functional and daily-living activities and in using assistive and supportive devices. Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributes to patient care conferences.

     

    Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs.

    ATTRIBUTES

    + Three (3) to six (6) months of previous experience preferred

    + Compassionate, kind, and patient-focused

    + Excellent communication and customer service skills

    + For ElderONE Employment: 1 year of experience with the frail elderly population preferred

    + Minimum Qualifications: Degree in Occupational Therapy, bachelors, masters or doctorate.

    + Current NYS Occupational Therapist License or limited permit as a Registered Occupational Therapist.

    + RESPONSIBILITIES

    + Evaluation & Development. Perform a complete initial evaluation of referred patients using evaluation protocols, standardized tests, and clinical observations; develop and implement treatment plans, including short- and long-term goals and methods to achieve them; adjust treatment plan and goals as needed

    + Documentation & Communication. Maintain complete documentation of patient evaluation, treatment, progress, and discharge status; counsel and teach patients and families; coordinate care plans with other involved health professionals, and discuss changes in treatment programs with MDs

    + Employee Development. Assist with the supervision and guidance of new personnel, assistants, students, and volunteers as assigned; develop and conduct in-service training programs

    PHYSICAL REQUIREMENTS:

    M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

     

    For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

    The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
